| What are your hours? We have office hours Monday through Friday. Monday 7:00 am - 5:00 pm Tuesday 7:00 am - 5:00 pm Wednesday 2:00 pm - 5:00 pm Thursday 6:30 am - 5:00 pm Friday 11:00 am - 1:30 pm |

| Where are you located? On the south west corner of Quail and Colfax in Lakewood. (2 blocks east of Simms/Union by the Post Office) |

| What insurance do you accept? We accept most insurance plans. |

| How do you handle deductibles? We collect an estimated deductible amount at the time of service. Most commonly we collect around $42. (until the deductible is met) |

| How much will treatment cost if I have no insurance? We offer a cash discounted rate for patients without insurance. Examination $35 - Treatments $45 Possible extra charge if acupuncture is performed. (all prices subject to change without notice) |
